Beneficiary, Dependant or Next-of-Kin Nomination Record
Populated beneficiary and dependant nominations held by superannuation funds and employers: binding and non-binding (including lapsing and non-lapsing) death benefit nominations under Superannuation Industry (Supervision) Regulations 1994 reg 6.17A (made for SIS Act 1993 s 59(1A)), employer group life insurance beneficiary nominations, and HR dependant, next-of-kin and emergency contact records. Each record ties an identified member or employee to named family members, their relationship to the member and, on death benefit forms, the share each person receives, witness details and dates of birth. Bulk member extracts with TFNs and balances belong to superannuation-fund-member-statement-bulk, and super identifiers (USI, member number) on their own belong to global-top500-096-superannuation-identifiers.

- Medium: nomination forms and dependant records are dense labelled forms, and the enforce tiers need a populated member field plus a named beneficiary with a relationship, which PDS text, policies and blank forms do not carry. Name and relationship shapes are loose, so HR contact lists and insurance correspondence with the same labels can reach 75, and a member date of birth can lift an emergency contact record to 85. 85/75 are heuristic evidence tiers, not measured accuracy; test values are synthetic records laid out like fund nomination forms and HR dependant records.

Death benefit nominations. You can make a binding death benefit nomination directing the trustee to pay your benefit to your dependants or legal personal representative. Known false positives
- Superannuation fund PDS documents, member guides and fact sheets that explain binding and non-binding nominations, witness rules and lapsing. Mitigation: Guidance names the fields without values and stays at 65 discovery; "this PDS / guide / fact sheet" wording and legislation markers are excluded from enforce tiers.
- Blank nomination forms and dependant / next of kin forms. Mitigation: Member and beneficiary fields reject blank lines, bracket placeholders and n/a / tbc values; fixture wording is excluded by the shared template-exclusion dictionary.
- HR policies and onboarding instructions about emergency contacts. Mitigation: No populated employee field and named contact with relationship, so these stay at 65; "this policy applies" wording is excluded.
- HR contact lists and insurance correspondence that name a member and a relative with the same labels. Mitigation: These are still personal information about family relationships; accept at 75 or tune the relationship list locally.
