Psychosocial Hazard Risk Register or Risk Assessment
Psychosocial hazard registers and risk assessments kept by a person conducting a business or undertaking under the Work Health and Safety Regulation 2011 (Qld) Part 3.2 Division 11 (s 55A meaning of psychosocial hazard, s 55B meaning of psychosocial risk, s 55C duty to manage psychosocial risks, s 55D determining control measures) and the Managing the risk of psychosocial hazards at work Code of Practice 2022 (in force 1 April 2023). Register rows record a hazard (job demands, low job control, poor support, aggression from customers, remote or isolated work), a risk rating, controls and an owner for a team or work area. Most rows are aggregate; the higher tier is reserved for rows that identify an affected worker or link a complaint, grievance or claim. The Code's own text and example register, blank registers and training material are decoys. Complements au-workplace-harassment-psychosocial-complaint, which covers individual complaints.

- Medium: the WHS Regulation Division 11 terms and the Code of Practice vocabulary are distinctive, and a populated rating plus hazard or control field separates a working register from guidance, but the Code itself publishes an example risk register with the same columns and training decks reproduce it, so guidance and training wording is excluded and blank fields are placeholder-guarded. Most registers are aggregate (team or work-area level), so 85 is reserved for rows that identify a worker or link a complaint, grievance or claim. Employee and complaint number formats are organisation-defined and loosely shaped. 85/75 are heuristic evidence tiers, not measured accuracy; test values are synthetic.

Known false positives
- The Code of Practice, WorkSafe guidance and Safe Work Australia material, including the Code's example risk register. Mitigation: Guidance exclusion on this Code / this Code of Practice / example risk register; enforce tiers need populated rating and hazard or control fields.
- Training decks and workshop exercises that fill in an illustrative register row. Mitigation: Excluded on learning outcomes, facilitator notes, training module, workshop activity, case study and the shared template-exclusion dictionary.
- Individual harassment or bullying complaints that mention a psychosocial risk assessment. Mitigation: These rarely carry a rating field; they are covered by au-workplace-harassment-psychosocial-complaint.
- Genuine registers that cite the Code by the words "this Code" (for example in a quoted requirement). Mitigation: Suppressed within the 300-character window; accepted false negative.
