QLD Ambulance Service Patient Care Record

Populated Queensland Ambulance Service (QAS) patient care records: electronic Ambulance Report Forms (eARF, earlier ePARF), paper ambulance report forms and paramedic patient care records or reports. They record the patient, the case or incident, chief complaint and mechanism of injury, vital-sign observations, treatment and medicines given, and the transport destination or hospital handover. Copies are requested by hospitals, police, insurers, lawyers and coroners, and travel outside the QAS clinical system. Boundary: hospital clinical handover sheets belong to au-qld-health-clinical-handover; police incident reports to qps-incident-report.

Medium: eARF and ambulance report form wording is specific, and enforce tiers need a populated patient and a populated clinical field. "Patient care record" is also used by aged care and hospital systems; those records reach 75 when they carry observations or treatment, which is health information about a person, and only reach 85 with a case number or QAS provenance.
