QLD Health Discharge Summary or Referral Letter
Populated clinician-to-clinician transfer documents: hospital discharge summaries, discharge letters, transfer of care summaries, GP and specialist referral letters and requests for specialist consultation. They carry patient identifiers together with the structure of a handover between treating teams: admission and discharge dates, discharge destination, referring, receiving or usual GP, and clinical content such as principal diagnosis, medications on discharge, reason for referral and follow-up arrangements. Boundary: au-qld-health-medical-record covers medical records generally and fires on the bare phrase "discharge summary" plus a patient identifier; this classifier enforces only when discharge or referral structure (dates, referring or receiving clinician) is also populated. Shift handover sheets belong to au-qld-health-clinical-handover; My Health Record documents to au-my-health-record-clinical-document.

- Confidence justification
- Medium: the document titles are standard across Queensland Health, private hospitals and general practice, and enforce tiers need populated patient and handover-structure fields, which guidelines, leaflets and blank forms lack. Field labels are matched by wording only, so a clinic letter that uses the same labels without the title stays at 65 and an unusual layout with unlabelled fields may be missed. 85/75 are heuristic evidence tiers, not measured accuracy.

Known false positives
- Clinical documentation guidelines, junior doctor training material and patient leaflets that name discharge summaries and referral letters and list their fields. Mitigation: They lack populated patient and handover fields, and guideline, patient-information, training and news framing is excluded from enforce tiers.
- Blank discharge and referral forms. Mitigation: Field values reject blank lines, underscores, brackets and placeholder words; fixture wording is excluded by the shared template-exclusion dictionary.
- Records requests and release-of-information letters that name a discharge summary. Mitigation: With no admission, discharge or referral date or clinician field they stay at 65; au-qld-health-medical-record covers them.
